21^st Century Health Equity Practice requires transformation not only in the way we conceptualize health and the determinants of health, but also how we respond as practitioners. In particular, we must consider factors beyond traditional concepts of healthcare and health behaviors, and look to create opportunities for health at the population health level, within a broad and historically informed social and structural determinants of health frameworks.
